Eligibility
Hospice is a concept of care for patients and their families. The care is based on the needs of the patients and families; i.e., the need for comfort, to be free from pain, and to receive spiritual and emotional support.
Hospice Support of Fauquier County, Inc. is a non-profit, volunteer organization which provides palliative and supportive care to terminal patients and their families in Fauquier County. Emphasis is on enabling the patient to remain at home if at all possible; however, Hospice Support serves wherever the patient may be.
Care is provided through teams of trained volunteers and an interdisciplinary professional staff, coordinating home and inpatient care, when necessary.
The program is supported by the community by contributions of time, services, materials, and monetary gifts, many of which are memorials to those patients we have served.
ADMISSION
- Patient and family must want Hospice Support.
- Patient is facing a limited life expectancy.
- Patient must reside in Fauquier County.
REFERRALS
A physician, minister, family member, a friend, a nurse, or anyone who sees the need may refer a patient and family.
